Field Trip Permission Form

This template may be used by any educational institution to obtain the permission of a parent or student (if above 18) to go on a trip that leaves the school grounds. This form is usually sent by the school with at least one (1) week before the excursion and the parents are usually told where the trip will be and what it will consist of.

Once authorized by the student (if above 18), the parent, or guardian the form holds the school harmless of any legal liability that may occur.
