Arkansas Living Will Forms and Templates

The Arkansas living will sets out a patient’s end of life treatment decision based on if he or she becomes permanently unconscious as defined by the Arkansas Rights of the Terminally Ill or Permanently Unconscious Act. This means that the form becomes legally acceptable upon the determination that the patient’s condition is incurable and that he or she is nearly impossible from coming to a stable thinking of the mind. Upon these findings the patient’s wishes as described will be carried out by the medical staff.
