Download Arizona power of attorney that allows an agent (also referred to as the “attorney in fact”) to act in a person’s best interest for a financial, health care, tax, parental authority, motor vehicle, or on a limited type of activity. Most of the documents must be signed in the presence of a notary public pursuant to State law.

  • Durable/General – This form allows you to pick someone to represent your financial best interests by allowing he or she to make any monetary decision in your shoes as long as it is in your best interest. With this document, you have the option for the period to last until a period of time or at the time of death or revocation. Must be authorized in front of a notary public in order to be legal.
  • Medical – Allows someone else to handle all health care decisions in the chance you may not be able to do so for yourself. The document must be authorized in front of a notary public in order to be considered legal. Note that the agent selected has a fiduciary duty to always act in the best interests and health well-being of the person being cared for.
  • Limited (Special) – For all financial related actions or decisions assigned to another person on a limited basis. The principal has the option to have the document last for a period of time, the specific task, or to continue ongoing until revocation or death of the person being represented.
  • Tax (Form 285) – Allows a person to choose someone else to handle their tax filing with the Arizona Department of Revenue. It is highly recommended that you select a person that is highly knowledgeable in the field such as a tax attorney or Certified Public Accountant (CPA).
  • MVD – Use this form to elect someone else to handle any type of Motor Vehicles Department action on a person’s behalf. Typically this will relate to having someone else register or get title for an automobile. In order to be considered legal it must be authorized in the presence of a notary public or MVD agent.
  • Parental – Allows a parent to grant authority over a child to someone else who must; reside in the State, be eighteen (18) years of age or older, and be of sound mind. The length of the arrangement cannot be for more than a six (6) month period unless the person granting the authority is in the military.

How to Cancel an Arizona Power of Attorney?

  • If the document is not a durable or medical form, death or incapacitation of the principal
  • Authorizing a new document automatically cancels any and all previous versions
  • Authorizing a Revocation Form

Whenever an arrangement has been cancelled be sure to notify all public and private institutions in order to notify that the representation is no longer in effect.
