Download the Standardized Mini Mental State Examination (SMMSE or “Folstein Test) to check the mental status of a patient by asking standard everyday question that the average person should know fairly easily and quickly. If a person is slow to answer most of the questions he or she may be developing a form of Dementia or some other type of memory loss syndrome or disease.

You can fill out the form by grading the patient on a one (1) to five (5) scale based on their quickness and accuracy of their answers. Any score of 27 or greater is the presence of a strong and healthy mind.
