The DS-82 form is needed if you require renewal for an expired passport or one that is about to expire. A passport that is about to expire usually means within 6 months (180 days).

How to Renew a Passport
Ask yourself the following questions;
- Your Most Recent U.S. Passport is:
- Is undamaged and can be submitted with your application;
- Was issued when you were age 16 or older;
- Was issued within the last 15 years; and
- Was issued in your current name or you can legally document your name change with original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change.
If you said no to any of the following you will have to go to a Passport Agency Office in person and pay the expedited fee.
Step 1 – Answer the Qualifying Questions – If you answer “no” to any of the first questions on the form, you will have to use Form DS-11 and appear in person.
Step 2 – Study the Instructions Gather Documents – If you will be able to use this form you will need to gather all of the required documents that will be needed to renew your passport.
Step 3 – Submit Documents, Forms and Fees – Once you’ve gathered all of the required documentation and provided all required information, send these along with any required fees payable to the Department of State to:
