The Form I-134 is an Affidavit of Support making it possible for you to sponsor an immigrant who is applying for a U.S. visa. This form shows that anyone applying for a visa would have an adequate amount of sponsorship (financial support) and will not become a “public charge” if approved to reside in the U.S. This means that the government would like to be certain that whomever you would sponsor, you would be willing to support entirely until which time the immigrant would have permission to work and support him/herself or until the person(s) who are only visiting in the United States return to their home land.

This form is to be filled out and filed at the beginning of the visa application process. The sponsor must file a separate affidavit for each applicant to be sponsored. For example, if you’re planning to sponsor a family of three, a form must be filled out for each respective family member.
How to Fill Out
Step 1 – Download Form I-34 and The Instructions – Once you have downloaded these pages, study the instruction prior to completion of the form.
Step 2 – Complete Your Sponsorship Form – Complete the entire form and gather any documentation prior to submission.
Step 3 – Oath and Submission of Form – Although no notarization is required it’s very important that you sign the oath or Affirmation of Sponsor. Once this has been completed, attach all required documentation and submit your information to USCIS.
