Wisconsin Parental Power of Attorney Form

Wisconsin Parental Power of Attorney Form (Delegating Guardianship of a Minor) – In pursuant to Statute 48.979, a parent of a minor may be able to choose someone else to handle all guardianship decisions on behalf of their child. The delegation lasts for a period of up to one (1) year. If the parent wishes for the guardianship to last longer than a year a new power of attorney form must be completed.
