Alabama Living Will Form

The Alabama living will or “Advanced Health Care Directive” or “Health Care Proxy” is a legal document that allows any adult to decide their fate in the chance they cannot speak for themselves due to mental conditions. Under Alabama law 22-8a-4, this form was created by the State legislature to assist patients in creating their end of life treatment decisions.
