Arizona Living Will Form (Health Care Directive)

The Arizona living will form allows a person to be able to make their end of life treatment decisions in the chance they should become in a state where they cannot represent themselves. After completing the form, it is recommended that the person make copies and hand them out to family members and any and all doctors that they may be associated with. Unlike a power of attorney form, this type of end of life decision puts the medical care in the hands of the document created by the patient and not in the person selected by the individual.
