How to Change Your Name In Colorado

  • Step 1

In order to start the process you have to get a background check done. You will have to go to your local law enforcement office to get this handled; you will have to get your  fingerprints printed, and you will get about 10 copies, which you will then need submit to get a background check through to the Federal Bureau of Investigation (FBI).

  • Step 2 

Here is the process in sending everything you need over to the FBI! Here is the application that you must fill out to send over to the FBI. With this application you should attach your copies of your fingerprints; and you should attach a check for $18.00 as it is a processing fee for the background check. Send all of this information to:

  • Step 3

Its now time to file a petition for a name change at the County Clerks office which is the Colorado Name Change forms known as the Petition for Change of Name (JDF-433). You will need to get your petition notarized with a local branch bank, along with the background check you received from the FBI; please make sure to mark it as Exhibit A. The filing fee to submit all this is $83.00.

  • Step 4

You will be given a hearing date after filing your petition; you will either be approved or not approved. If approved you must make your name public in the newspaper 3 times or more in next 21 days thereafter being approved. To get this done you must submit the Colorado Public Notice of Petition for Name Change (JDF-427). You will have another hearing; but you are now in the final steps of it all. This is another form you will need for your second hearing Colorado Name Change Decree (JDF-448); this will get your official certificate for the name change so you may give to all legal institutions to change your name officially.
