The Illinois living will is a form that a patient may use to control his or her health care decisions at end of life stages. If a licensed physician has determined that a patient is terminally ill and unconscious and that the measures being taken are only delaying the inevitable, then the patient, through a living will, may withdraw the death delaying measures.

Once the form is completed, it is recommended that it be notarized or signed with two (2) witnesses present.
