The Kentucky living will statute (Code 311.625) is a form designated by the attorney general that permits a person to be able to select their end of life treatments and care if he or she should become in a state of mind where there is no cure. The patient will usually be comatose or incapacitated and through this form the patient will be able to select organ donation options as well as if the medical facility should keep life prolonging treatments on-going.

Once the person has decided on their options he or she will want to have the form signed in front of a notary public (recommended) or two witnesses. The form should be distributed to the patient’s primary care physician and to any and all medical facilities he or she is a patient.
