The Louisiana living will is a legal document that allows a person residing in the State to be able to choose their end of life treatments in regard to feeding tubes and respirators prolonging their life. If the person feels it is in their interest to cut life sustaining measures as described in Louisiana law (§1299.58.1), then he or she may do so by filling out the following form and having at least two (2) witnesses for the signature.

• It is recommended that loved ones and friends receive a copy of this as well as the primary care physician for the patient.
