The Marriott credit card authorization form allows the payment of hotel rooms or services through the phone, fax, or E-Mail. This is commonly used when corporations pay for their employees lodging on business related trips. This document is valid for the use of a one-time payment at the end of the stay and the company may set a limit on how much may be charged.
Marriott must obtain the following cardholder details in order to authorize:
- Cardholder’s Name/Company
- 16 Digit Card Number
- Expiration Date
- Billing Address
- Security Code
- Telephone Number
- Signature of authorized person.
*Do not send a scanned copy/picture of the credit card.
