The Maryland living will, or Advance Directive, is a form that allows a person to choose an agent to make medical decisions on their behalf (“agent”) as well as make end of life treatment options. The options include turning off feeding tubes, breathing machines, or any medical instrument that prolongs a persons life from an uncurable incapacitation. If the medical staff taking care of the patient deems valif that there is no definitive cure for the person’s condition, then the form will come into effect. Also, if the patient elected to donate their organs they should be taken into consideration by the medical team.

To make this form legal the patient must fill in the form and sign with the parties’, witnesses, and a public notary (if necessary).
