MA Living Will

The Massachusetts living will form, also referred to as a “Advance Directive”, is used by residents to make their end of life treatment and organ donation options. The patient will fill out the form and write in their basic wants/needs and also have the option to let an agent be able to step in and make decisions on their behalf.
