Utah Durable Power of Attorney Form

A Utah Durable Power of Attorney Form grants a resident to transfer power (See powers granted by State in 75-5-311) to handle one’s finances to another person. It is recommended that the person chosen is a family member or friend as he or she will have the unlimited use to act “in your best interests” for any monetary related decision within the State. Must be signed with a Notary present in order for it to be accepted at most public and private institutions.
