Washington Revocation Power of Attorney Form

A Washington Revocation Power of Attorney Form is a document created and filled out by the Principal of an existing power of attorney form. Once executed, the original power of attorney form will no longer cease to be valid. The Principal should also notify all parties involved in the original power of attorney form that it has been cancelled.
