These instructions are for adults who don’t like their name, getting a divorce, or have just gotten married. The instructions are in accordance with the Arizona Statute 41-164.

  • Step 1

First you must fill in the name change forms which are as follows;

Be sure to have your signature witnessed by a public notary. To get your petition notarized all you have to is go to a branch bank; and if you are actually with a branch bank you can get the notarized services free. You must be 18 years of age or higher to fill out a petition for a name change. If just married or divorced than it is required that you have the original information for both.

The forms to file may be different for every court, be sure to check with the clerk’s office to make sure the forms you have are the ones required.

  • Step 2

You are now ready to file your petition for your name change; be sure to have all personal information like you social security card, birth certificate, and your state identification, importantly make sure they are all original. You will have to file your petition at your local county clerks office; you will than get a hearing date from them. How they go about contacting you is either by email or they may contact you through the phone; at this point all you can do is wait to hear from them.

  • Step 3

Its hearing time; at the hearing make sure to have your “order changing name”; the official form, so that the judge may authorize it. Now its time to announce; its under Arizona’s State Law that you must legally let all institutions know for name change meaning DMV; Social Security Office; Insurance Companies; etc.
