Credit card authorization forms are used to process a payment when the cardholder is not able to be present. The form is filled-in with all the information about the cardholder including the number, name on card, expiration date, and the security code. After the cardholder signs the form it is faxed to the processor and the card may be charged.

How to Write

This is usually done through a fax machine where the vendor accepts the following credit card information in order to process the transaction such as;

• Sixteen (16) Digit Credit Card Number
• Name on Card (Not the Entity)
• Expiration Date
• Security Code – Last 3 digits on the Back (For American Express it will be 4 digits on the front)
• Credit Card Billing Address and Telephone Number
• Cardholder’s Signature including Date – This does not have to be an original and may be sent through a standard fax machine or electronically through e-Fax.
