Free Annual Credit Report Request Form

The annual credit report request form is provided by the Fair Credit Reporting Act (FCRA) allows every resident of the United States to be able to obtain a free credit report every year from Equifax, Experian, or TransUnion.

How to Get a Free Annual Credit Report

You may make this request in the following ways:

• Online – The website www.annualcreditreport.com will allow you to process and view your credit by registering/logging in and verifying your identity by answering questions about family and where you have resided in the past. Although you may be asked, you do not have to sign up for a membership in order to view.
• Call – You may call the free credit hotline at 1-877-322-8228 and speak to an agent over the phone who will be able to give you details about your score and what may be affecting it.
• Mail – You may complete and send in the Annual Report Request Form by sending it through the mail. The process will take anywhere from 2 to 6 weeks as processing can take up to 15 days. Complete the form and send to the following address:
