Use the Hyatt credit card authorization form to accept a customer’s credit card at any location worldwide. Typically this is used when a person needs to make a payment with a credit card that is not physically with the individual which is common with business travelers.

If for some reason the card is not working electronically or if a payment has to be made over the phone, the details should include:

• Name
• Credit Card Number
• Expiration Date
• Statement Mailing Address
• Statement Telephone Number
• Cardholder and Guest’s signature.
• And both sides of the card must be photocopied.
