The VA Form 21-526EZ is the Application for Disability Compensation and Related Compensation Benefits which is required by the federal government when a veteran of the armed forces requires monetary aid as a result of being in the military. How to Use
Veterans may claim these benefits if they have a disability that was caused by their military service, the time in the service is connected to or has made a disability worse, has caused an additional disability, the disability gained while in service will prevent them from gaining meaningful employment of any kind, the disability was caused or worsened by VA medical treatment, vocational rehabilitation, or work therapy and/or the service connected disability has caused one to be bedridden, in need of aid or otherwise confined to their home. The form itself is only a couple of pages leaving the bulk of the document to give instructions regarding additional supporting paperwork or forms. The VA Form 21-526EZ will require proof of the disability and proof the veteran was in the service, especially if the time served is within a year. The form requires one to submit his/her disabilities, service information, any VA Medical Center where treatment was administered, service pay, personal information, the applicant’s signature, and in the case where the Veteran signs with an ‘X’, the signature of a witness to the veteran’s signature.
It should be noted that submitting this form may affect retirement pay. That is if one is receiving retirement pay it is entirely possible that the judgment regarding VA Form 21-526EZ will be deducted from the retirement may. Thus, if the applicant is eligible for x amount of dollars per month in disability payment then x amount of dollars will be subtracted from the applicant’s retirement check. There is a box on section three however that will prevent this but it may sacrifice any benefits relevant to VA Form 21-526EZ. Thus, it is important to weigh all the factors when collecting retirement.
The applicant may also have to submit additional forms with this form. For instance if he/she is claiming dependents, individual unemployment, specially adapted housing or special home adaption, an auto allowance, or veteran/spouse aid and attendance then additional forms will be necessary. The forms for each instance are listed in the instructions that accompany the VA 21-526EZ.
The Veterans Affairs Department names the Fully Developed Claim Program (the FDC Program) as the most expedient way of submitting a claim and will provide contact information on the VA 21-526EZ.
