A Georgia living will or “Advance Directive for Health Care” is a legal form that allows a person to set their future health care preferences if the worst should happen and they cannot make decisions for themselves. Unlike a power of attorney form which grants the rights to an individual, this form makes pre-existing decisions that cannot be changed and must be carried out by the medical facility and staff that is taking care of the patient. It is important that copies of this completed, signed, and notarized form be distributed to the health care representatives that will be in charge of the individual if this event should occur.
