How to File
Step 1 – Complete the Form – Fill in the blanks either on your computer or after printing and handwriting in black ink. A docketing statement must accompany one of the following forms below;
- Choose One
- Domestic (Most Common) – For all companies that will be located within Pennsylvania.
- Foreign – For all companies located outside the State.
Step 2 – Pay the Fee – A fee must be attached to the form. Make checks payable to the ‘Commonwealth of Pennsylvania’.
- Domestic – $125
- Foreign – $250
Step 3 – Mail – Send the completed forms and send the fee to:

Tips
- Operating Agreement – Download and complete this form if you own a business on your own and would like to protect your entity status.
