How to File
Step 1 – Application – Complete the articles of incorporation form either online or by downloading the application. It is a simple two (2) page application that will take approximately ten (10) minutes to complete.
When filling out Item 2 on the application, you will need to input one of the following codes:
- DP – Domestic Profit (Most Common)
- PA – Domestic Professional
- FR – Foreign Profit
- DBA – Foreign “Doing Business As”
- DP – Domestic Non Profit
- NF – Foreign Non-Profit
Step 2 – Attach fee – You must attach the filing fee of $125 (non-profit $75) and send the completed form to the Secretary of State’s address located at:

- Apply for an EIN Number – If you are seeking to open a bank account with your entity you will need to get a tax identification number for it. You can download the form and apply in the mail or apply online.
