Instructions on How to File
Option 1 – Apply Online – If you seeking to create an LLC that is located within the State of Ohio then you may form an LLC online. The cost will be the same as the paper filing ($125).
Option 2 – Printable Application – Choose the form below that matches your desired interests;
- Domestic (Most Common) – For all standard formations.
- Foreign – For LLC’s that have a professional designation such as a law or medical office.

Tips
- EIN Number – In order to register for a bank account the company will need to obtain an Employment Identification Number from the IRS. You may apply online (Free service and will take about 10 minutes where you will get the number at the end of the session).
- Operating Agreement – If you have partners in an LLC you want to make sure that you protect yourself by stating exactly the terms of the company in this agreement. This form will detail who has specific ownership as well as other business details.
