The Arizona articles of organization forms are used as the official application to create a new limited liability company in the State. The form, along with the filing fee ($50 for Domestic and $150 for Foreign entities), Cover Sheet, Member Structure Agreement, and Registered Agent Acceptance must be sent to the Secretary of State’s office.

How to File
Step 1 – Download and complete one of the Articles of Organization document along with the following forms;
- Domestic (L010) – For all new LLC’s that will be located inside the State
- Foreign (L025) – For all existing LLC’s that are located outside the State
After completing the articles the following forms must be attached;
Step 2 – Send all documents to the following address:
Arizona Corporation Commission,
Corporate Filings Section,
1300 W. Washington St.,
Phoenix, Arizona 85007
Tips
- EIN Number – It is required that if you will be planning on creating a bank account that you register for an EIN number. This is a reference number the IRS uses much like a Social Security number for personal use. You may only apply Monday thru Friday 7am – 10pm.
- LLC operating Agreement – Every attorney will recommend having an Operating Agreement to protect your personal assets (if a Single Member LLC) and to protect yourself from other members (if a Multi Member LLC).
